Your not really cured of the Eagle's poison. Jin just overcome his trauma by accepting his father's death was not his fault

It’s because Iki island was set after the main story. Iki was an expansion and it was an update for “Ghost Of Tsushima (DIRECTORS CUT) so you were supposed too finish the main story and then go to Iki but you can go to Iki after completing act1. A line when you go to Iki is that Jin Sakai said “Koutan Khan Is Dead” Indicating that it’s set after the main story.

It is strange I was wondering around bitter hills garrison and heard the line, “ The father and his child huddled together”. Just like in the hallucinations on iki island.
