This actually almost happened once. Charles Manson attempted to strangle the judge presiding over his case, afterwards, the judge kept a gun within arms reach for the remainder of the trial, despite it being against the law to do so.

For those who don't know, recently a man who was being given his sentence by a judge just lunged at her completely clearing her bench/desk and tackling her to the ground. She hit her head in the fall, he got a couple hits in, and tore some of her hair out before the guards got him off her. He later apologized to the marshals that was there for the fact that they had to see that. Obviously not including the judge in said apology.

My father's best friend was sentenced for throwing the prosecutor at the judge. Yes. A prosecutor. At the judge. He just lifted the prosecutor off the ground - he is freakishly big and strong - and just threw him at the judge.
