How To Complete a Form 433-F

This form is used for individuals who want to set up a payment plan with the IRS and who do not qualify for a streamlined payment plan (balance under $50,000.00 that can’t be paid in full within 6 years). In some cases, it may also be used for taxpayers who are assigned to the ACS division, and owe less than $250,000.
This form IS NOT for individuals working with a Revenue Officer, who will require a Form 433A or Form 433B.

Hi Bob, two questions if you don't mind answering: Are you required to list every credit card? As a senior widow, I had to get credit cards in order to build my credit score, so it's not frivolous debt, but it has added up. I hope it does not work against me in trying to resolve the balance. I've been on an installment plan for 4 years, so I will be submitting this form to show that I have no "disposable" income and hopefully get the remaining balance shelved due to financial hardship. And my other question is if a friend is helping with a small part of your rent payment, do you list on the form just the amount you pay or do you list what the total rent is? Thank you!
