After the Cold War ended almost 90% of the nuclear weapons were destroyed. Many of these weapons yields were in the megaton range because of inaccuracies in guidance and were ground bursts, causing massive amounts of fallout and radiation. Today, what's left are mostly in the kiloton range ( 120 - 150 ) air burst and designed to destroy cities with blast and heat. Still less than ideal but most of the radiation disappears into the upper atmosphere. Japanese citizens in Hiroshima and Nagasaki were able to walk around their cities shortly after the blasts and are still occupied today. These were air burst weapons. The Controversial Science of Nuclear Winter by Neil Halloran on YouTube is well worth a watch. Not every missile control officer is going to fire his weapon...Stanislav Petrov is an example. Last year in China it was found that many of the hatches on their land based ICBM silos could not be opened and the missiles had water instead of fuel in their tanks. Many weapons will fail to launch like the last two Trident firings from the British Vanguard class submarines. The Vanguard subs are British but the missiles are American and are maintained in the USA. Both US/UK subs draw their missiles from the same stockpile. Worrying but strangely comforting.

Potassium iodide is for NPP accidental leaks, not for nuclear bombs. Compared to radiological accidents in NPPs, a nuclear bomb creates a negligible amount of radioactive iodine, to the point that if potassium iodide were to be helpful, you'd be dying of radiation poisoning from every other radioactive isotope that's not iodine.
